Q: Who may receive hospice care? A: Hospice care is specifically for people with a life limiting illness who meet the following Hospice Guidelines:
Q: How much does Hospice care cost? A: Hospice can be reimbursed by Medicare/Medicaid, Champus, VA, and most private health insurance plans. However, service is provided to those who meet admission guidelines regardless of their ability to pay.
Q: Can I still use my own physician? A: Yes, you continue to use your own physician. Your physician's care plan will be implemented by a Hospice nurse, social worker, a chaplain, home care aides and trained volunteers. If you do not have a local physician, the Hospice medical director is available to assist with your care.
Q: What can Hospice volunteers do for me? A: Hospice trained volunteers provide periodic relief for the primary caregiver, socialization for the patient, and often just listen.
Q: How often do Hospice staff visit? A: Hospice care is tailored to individual needs. Hospice personnel are on call 24 hours a day.
Q: Who should I contact to begin Hospice care? A: Ask your physician to call Sacred Journey Hospice, or contact us directly at 678-583-0717. We will discuss admission procedures with you.
