Founded by a group of nurses and a physician assistant, Sacred Journey Hospice has been providing quality hospice care to patients in south metro Atlanta since 2002. Every patient and family is assigned a care team that works under the direction of the our Medical Director and in collaboration with the patient's physicians to give the best possible care.
Our interdisciplinary team of healthcare professionals, home health workers, counselors, chaplains and trained volunteers ensure that each individual receive the quality of care they deserve. At Sacred Journey, we also consider the patient and family as members of the team and encourage their involvement in making decisions regarding their care.
