When the focus shifts from cure to care... it may be time for hospice.

Sacred Journey Hospice works with physicians throughout metro Atlanta to offer patients and their families a resource for the physical and emotional support they need when facing a life-limiting illness. With skilled nurses, healthcare workers, counselors, social workers and volunteers available 24 hours a day, hospice can provide a valuable support structure to complement your care.

When you refer a patient to Sacred Journey Hospice, you offer them an alternative in care that will allow them to live with dignity in the company of their loved ones during what may be their final moments of life. The focus of hospice is on pain and symptom management and providing quality, compassionate care for those with a life-limiting illness.

Whether at home, in a residential facility, or at our inpatient facility, we can work with you to implement a Plan of Care with your directives. Our staff Medical Director can also assist when needed.

Hospice care can also reduce out-of-pocket expenses for your patients, and generally does not require any out of pocket expense for the patient. As the attending physician of a hospice patient, you will be reimbursed monthly by Medicare, Medicaid or the patient's private insurance plan for your patient care and plan oversight.

The Four Levels of Hospice Care

Routine Home Care is provided in the comfort of the patient's home or in a residential facility such as an assisted living facility or nursing home. Services are administered on an intermittant basis according the the Plan of Care.

Continuous Care is provided when the patient is in crisis and symptoms cannot be managed with routine home care, though the patient remains in a residential setting. Continuous care may be implemented to alleviate the need to transfer the patient to an inpatient facility or hospital.

Inpatient Care is designed for short-term accute needs in an inpatient facility or hospital. Typically the patient's symptoms have progressed to a stage that they cannot be controlled in a residential setting.

Respite Care provides short-term relief to a the patient's caregiver by transferring the patient to an inpatient facility or hospital for up to five days.
