Hospice Services

Four Levels of Hospice Care
Level 1: General Inpatient
Location: Contracted Skilled Nursing Facility or hospital
Duration: Short-term, average length of stay is hours to days
- For patients experiencing uncontrolled symptoms such as pain, respiratory distress, vomiting, bleeding
- The physician determines when a patient is stable for transfer and hospice social workers coordinate transfer to home, assisted living facility, or a nursing home
Level 2: Continuous Care
Location: Home, assisted living facility, or nursing home
Duration: Short-term, average length of stay is 24-48 hours
- For patients who required around-the-clock support during episodes of acute medical need, ensuring comfort and symptom management at home
- The physician determines a nurse is required to stay with the patient for a few hours to frequently medicate and reassess any uncontrolled symptoms
Level 3: Routine Care
Location: Home, assisted living facility, or nursing home
Duration: Long-term
- For patients with terminal diseases in need of a care plan
- The primary caregiver, family, assisted living facility, or nursing home provides care and medication while the hospice team oversees the plan of care
Level 4: Respite Care
Location: Contracted Skilled Nursing Facility or hospital
Duration: Maximum 5 days
- For patients whose caregivers/family members need temporary relief (vacation, emergency, etc.)
- Respite is scheduled by a hospice social worker

Counseling Services
Grief is a normal human response to serious illness and loss. We believe it’s important for individuals to seek professional guidance to understand how to cope with the emotional, social, spiritual, and financial stressors that come with life-limiting illness and the loss of a loved one. Our bereavement team offers individual and group support programs – including a Grief Recovery Program that meets weekly for 12 weeks. Additionally, we offer spiritual care with trained volunteer chaplains who offer services such as counseling, prayer, sacred writings, communion, and more.
