The Four Levels Of Hospice Care
Level 1: General Inpatient
Short-Term: Average length of stay is hours to days
Location: Contracted Skilled Nursing Facility or hospital
- For patients experiencing uncontrolled symptoms such as pain, respiratory distress, vomiting, and/or bleeding
- The physician determines when a patient is stable for transfer and hospice social workers coordinate transfer to home, assisted living facility, or nursing home
Level 2: Continuous Care
Short-Term: Average length of stay is 24-48 hours
Location: Home, assisted living, or nursing home
- For patients who require around-the-clock support during episodes of acute medical need, ensuring comfort and symptom management at home
- The physician determines when a nurse is required to stay with the patient for a few hours to frequently medicate and reassess the uncontrolled symptoms
Level 3: Routine Care
Long-Term
Location: Home, assisted living, or nursing home
- For patients with terminal diseases in need of a care plan
- The primary caregiver, family, assisted living facility, or nursing home provides care and medication while the hospice team oversees the plan of care
Level 4: Respite Care
Short-Term: Maximum 5 days
Location: Contracted Skilled Nursing Facility or hospital
- For patients whose caregivers/family members need temporary relief (vacation, emergency, etc.)
- Respite is scheduled by a hospice social worker
